Prepare For Your Visit

Insurance, Co-Payments,
and Deductibles

As you may know, the nature of the insurance industry has changed dramatically over the past few years.  Many policies now require higher co-payments, but not everyone is aware that deductibles have risen as well.

There are several steps that patients can take in advance of their visit that will make their podiatry appointment go smoothly:

First, please check with your insurance company to be sure that services provided by Dr. Boudreau will be covered under your insurance policy and that you know whether any co-pays or deductibles will apply to your visit.

If you are a new patient, please take the time to print out and complete the new patient information form prior to your appointment.  Remember to bring your completed form to the office on the day of your visit.  If you cannot print out the form, please arrive 15 minutes before your appointment to complete the form at our office.  It is important that you bring a list of the medications you are currently taking so that you can fill out the form completely.

If possible, bring any relevant x-rays or copies of relevant test results to your appointment for the doctor’s review.

Please have your insurance card(s) with you at the time of your visit.  We will need to make a copy for our records.

The doctor will want to know who your current primary care physician (PCP) is.

All patients should be sure to have a current referral from their primary care physician if their insurance company requires a referral to see a specialist.  

If you are seeing the doctor for a toe nail problem, please be sure to remove toe nail polish prior to your visit.  

Please be sure to bring or wear the shoes that you most often wear on a daily basis.

Finally, it is easier to remove socks in preparation for your exam than it is to remove full-length hosiery.  Please dress your feet appropriately on the day of your appointment.  When the doctor has easy access to your feet, it can make your foot examination and treatment go more smoothly.
